It’s possible for a sentence to have multiple verbs with multiple subjects, and that’s what’s going on here. The MAIN sentence is: Each must have vaccinations. The subject is, as you noted, each, and the verb is must have.

There’s another verb in the prepositional phrase. The full prepositional phrase is: [of the students who plan to go on the class trip]. There, completely contained within the prepositional phrase, is the verb plan, with its subject, students
